The Challenge

You're Facing One of the Hardest Decisions of Your Life

And you’re probably doing it without the information you actually need.

😰

You’ve toured facilities and felt that sinking feeling in your gut—but everyone says “it’s time.”

🤷

You don’t know if home care is realistic for your parent’s specific conditions—Parkinson’s, Alzheimer’s, stroke recovery, multiple chronic issues.

💸

You’re confused about costs. Is home care more expensive? Less? You’re comparing apples to oranges with no clear picture.

😔

You’re afraid of making the wrong choice—and living with regret if your parent declines rapidly after placement.

Get the Clarity You Need to Decide with Confidence

The Home vs. Facility Assessment gives you a complete clinical picture of your parent’s situation—and exactly what it would take to help them thrive at home.

Comprehensive Health Review

I review your parent's complete medical picture—diagnoses, medications, recent hospitalizations, cognitive status, mobility, and safety concerns. Nothing is overlooked.

Home Environment Evaluation

If appropriate, I assess their living situation—safety hazards, accessibility needs, space for care equipment, and what modifications (if any) would be needed.

Family Situation Analysis

We discuss your family's capacity, concerns, and boundaries. What support can you provide? What's realistic? What are your non-negotiables?

Personalized Cost Comparison

Not generic numbers—YOUR numbers. Facility costs for their level of care vs. comprehensive home care, broken down by month and year.

Your Assessment Is Conducted by Emma Soy, RN

I’ve been a registered nurse for over 35 years, working across major hospital systems, home health agencies, and healthcare organizations.

I specialize in complex neurological conditions—Parkinson’s disease, Alzheimer’s and dementia, stroke recovery, and cases involving multiple chronic conditions that other agencies say “need a facility.”

As Founder and CEO of Gentle Shepherd Care, I’ve helped dozens of Charlotte families navigate this exact decision. Some became our clients. Others chose facility placement—and I helped them find the right one.

“My job isn’t to sell you home care. It’s to give you the clinical clarity to make the right decision for YOUR family—whatever that looks like.”
Questions

Frequently Asked Questions

How long does the assessment take?

+

Plan for 2-4 hours total. This includes reviewing medical records you provide beforehand, the consultation itself (phone, video, or in-person), and preparing your written report. This isn’t a quick sales call—it’s a thorough clinical assessment.

What if you recommend a facility instead of home care?

+

Then I’ll tell you that—honestly and clearly. I’ll explain why, what type of facility to look for, and what questions to ask. My goal is to help you make the right decision, not to sell you services. About 20% of assessments result in me recommending facility placement because it’s genuinely the better option for that family’s situation.

Is the $497 applied to services if we become clients?

+

No. The assessment is a standalone clinical service—you’re paying for my expertise, time, and the detailed report you receive. If you later decide to become a Gentle Shepherd Care client, those are separate fees. This keeps the assessment objective and ensures you’re getting unbiased guidance.

What do I need to prepare before the assessment?

+

After booking, I’ll send you a questionnaire asking for: your parent’s medication list, names of current doctors/specialists, list of diagnoses and conditions, recent hospitalizations or major health events, and your main concerns and questions. The more information you provide, the more valuable the assessment will be.

Can other family members join the assessment?

+

Absolutely—and I encourage it. Having siblings or other decision-makers involved helps everyone get on the same page. The written report is also designed to be shared with family members who couldn’t attend.

Do you offer virtual assessments?

+

Yes. While in-person assessments (including a home visit) are ideal for complex cases, I can conduct phone or video assessments for families who prefer that option. We’ll discuss which format makes the most sense when you book.

What's your cancellation policy?

+

Full refund if you cancel more than 48 hours before your scheduled assessment. Cancellations within 48 hours can be rescheduled once. No-shows are non-refundable. Life happens—just communicate with me and we’ll figure it out.
