Changing Chronic Care
from Illness to Wellness:
A Full Body Approach

Gentle Shepherd Care is transforming lives in Charlotte of those with Alzheimer’s, Parkinson’s, Multiple Sclerosis, and Lewy Body Dementia through holistic, personalized care that nurtures the mind, body, and spirit.

Imagine if...

  • Your loved one felt empowered again, regaining a sense of control over their daily life.
  • Instead of decline, you could see them experience a renewed sense of energy, dignity, and connection.
  • They have a life where both you and your loved one feel supported, not just in managing symptoms, but in living a fuller, richer life.

Life had always been full and vibrant for David, a retired architect who spent his days hiking local trails and sketching intricate designs in his notebook. But then, things started to change. He began losing his balance on those same trails, stumbling over roots that were once so familiar.

For David and his family these concerns created:

  • Fear of losing the person they know.
  • Helplessness as daily tasks are becoming daunting.
  • Frustration that care doesn’t address improving vitality and a happy life.

Introducing the Gentle Shepherd Care Longevity & Vitality Program - A Plan for Success in Care

Is Watching Your Loved One Struggle Becoming Overwhelming? Do you want more life for them? You’re in the right place!

The Longevity & Vitality Program: A Comprehensive Approach to Chronic Wellness Care

Beyond Healthcare to Focus on Wellness—A Program Tailored to Each Individual’s Unique Journey

What is it?

A 3-month chronic care program for persons with Alzheimer’s, Parkinson’s, Lewy Body Dementia, or Multiple Sclerosis.

Who created The Longevity and Vitality Care Wellness Program?

Our very own Founder & Registered Nurse, Emma Soy, RN, BSN, MA, after she saw a clear need in Charlotte for a structured program offering support for clients managing conditions like Parkinson’s, Multiple Sclerosis, Lewy Body Dementia, and Alzheimer’s, to focus on practical improvements in daily life and maintaining their independence.

Why did Emma create this program for you?

A Personal Note from Emma:  After more than 30 years of dedicated service as a nurse, I founded Gentle Shepherd Care in 2019 with a vision to redefine elder care. My journey began with a simple mission: to help older adults age gracefully in place, inspired by the fast-growing needs of the Baby Boomer generation. However, I quickly recognized a gap in the standard of care. Traditional elder care often focuses on managing daily activities (ADLs and IADLs) and relies heavily on medication for those with chronic illnesses like Parkinson’s, MS, Lewy Body and other dementias, or Alzheimer’s. But this approach, I realized, often just observes decline rather than enhancing quality of life.

I preferred to take a holistic approach to aging by addressing the markers of aging and focusing on personalized care that goes beyond medication. Our goal is to help clients optimize their health, slow the progression of chronic diseases, and thrive in their golden years. We are committed to fostering independence, dignity, and vitality for each person who enters our program of wellness.

Who is a good candidate for the Longevity and Vitality Program?

  • Older adults with chronic conditions such as Parkinson’s, Alzheimer’s, Multiple Sclerosis, Lewy Body Dementia. We also provide specialized care programs for other neurological, cognitive, or physical chronic conditions including arthritis, cardiovascular disease, and many more.
  • Individuals experiencing reduced mobility, balance issues, or muscle weakness as part of aging.
  • Seniors who want a proactive approach to slowing the aging process and maintaining independence.
  • Families searching for a comprehensive elder care solution that goes beyond standard medication and daily task management.

What are the pillars of the program?

  1. Personalized Medical Support: Customized care plans based on regular assessments.
  2. Physical Vitality: Exercises to improve strength, mobility, and coordination. Assistance with healthy nutrition, sleep management, and mobility.
  3. Cognitive Resilience: Brain health exercises to support memory, focus, and clarity.
  4. Emotional Wellness: Companion care and engaging activities to boost mood and reduce anxiety. Family education to keep everyone included.
  5. Holistic Care Coordination: Collaboration with families, healthcare providers, and therapists for seamless, integrated care.

How do you create a personalized care plan for each client?

We start with a thorough assessment that includes evaluating the client’s physical, mental, and emotional health, as well as their nutrition and vitamin needs, hydration levels, and sleep patterns. Based on these findings, we develop a customized care plan tailored to their specific needs and goals. The care plan becomes our working document, and we use our professional expertise to regularly review and adjust the plan to ensure it continues to meet current and evolving needs.

Does the program work alongside other medical treatments?

Absolutely. The Longevity and Vitality Program is designed to complement existing medical treatments. We work collaboratively with healthcare providers, such as doctors, physical therapists, occupational therapists and others to ensure the care plan that supports all aspects of the client’s health.

How does the program support clients with chronic pain?

Our program incorporates gentle, non-pharmacological pain management techniques, such as mobility exercises, relaxation techniques, and pain-relieving dietary recommendations. We focus on managing pain in ways that don’t rely solely on medication, allowing clients to stay active and comfortable while reducing reliance on pharmaceuticals.

What types of activities are included in the mental and emotional wellness portion of the program?

A range of activities designed to promote cognitive stimulation, social engagement, and emotional support are available. Examples include memory exercises, arts and crafts, light physical activities, music therapy, outdoor activities, and companionship visits. These activities help reduce isolation, improve mood, and maintain cognitive function.

How does Gentle Shepherd Care ensure clients stay hydrated?

Hydration is a vital part of our program. Our caregivers regularly encourage and monitor water intake, adjusting fluid needs based on individual health requirements. We use simple reminders, incorporate hydration-rich foods when appropriate, and educate clients and families on the importance of hydration in maintaining physical and mental well-being.

Are there specific measures in place for clients’ safety and fall prevention?

Yes, safety is a primary focus! Our program includes supervised mobility exercises and balance training to improve strength and stability, which helps prevent falls. When reviewing a client’s living environment, we often identify and recommend adjustments or modifications to eliminate potential hazards. During mobility activities, our team ensures consistent supervision and support to promote safety and confidence.

Why should I turn to Gentle Shepherd Care for care wellness support?

Because we understand what you are going through and can help you manage. Our entire focus is on your wellness in several areas:

  • Specialized Neurological Care: Our program is specifically designed for conditions like Parkinson’s, MS, Lewy body dementia, and Alzheimer’s, offering customized support to enhance quality of life.
  • Holistic, Personalized Approach: We tailor each care plan to the individual, ensuring the most effective, compassionate care possible.
  • Experienced, Compassionate Caregivers: Our caregivers are professionally trained to manage neurological symptoms with empathy, patience, and skill.
  • Focus on Lasting Wellness: Beyond symptom management, our program aims to equip clients with strategies to sustain wellness and independence.
  • A History of Success: We have been able to guide individuals in Charlotte to longevity and vitality on a regular basis and we want to see everyone in this community receive that same opportunity.

What are the costs associated with The Longevity and Vitality Program?

The cost varies depending on the level of care and frequency of visits required by each client. We offer a range of packages tailored to individual needs, and pricing is discussed during the initial consultation. We strive to create an affordable care plan that provides value and meets the family’s budget.

What is the minimum commitment required for the program?

We do not require a long-term commitment. Clients can engage with our services on a month-to-month basis. However, we often recommend completing the first 3 months as the program builds up to provide full solutions. It does not work to just apply everything immediately, so the 3-month timeframe makes it comfortable for the client and the family.
