Social workers provide essential support to aging individuals by assessing their needs, advocating for their rights, and facilitating access to necessary resources. They help understand the unique challenges faced by each individual, including physical health, mental well-being, and social support systems. They collaborate with the aging person and their family to advocate for them and support their care.
Additionally, social workers offer emotional support and counseling to help aging individuals cope with life transitions, loss, and changes in abilities. They know about and share community-based programs and activities to promote socialization and engagement, and provide education on serious topics such as chronic illness. In times of crisis or end-of-life care, social workers offer compassionate guidance and assistance to ensure dignity through the process. A Geriatric Care Manager, is an advanced professional who is an integral part of the care team. They manage care and advocate for older adults and their families. Their primary role is to assess, coordinate, and monitor the comprehensive care needs of aging individuals. All of our services start with a thorough assessment of the individual’s physical, mental, and social needs, and from that we develop a personalized care plan to address these needs effectively.
They act as the central point of contact for the elder, family members, healthcare providers, and other professionals involved in the individual’s care. They often join clients in healthcare appointments or hospitalizations, arrange for professional services, assist with transitions in care, monitor care needs, and also anticipate future needs.
Additionally, they provide support and guidance to family members, helping them navigate complex healthcare systems, understand available resources, and make informed decisions about their loved one’s care. Overall, their focus is promoting independence, dignity, and overall well-being for older adults while providing peace of mind for their families.
A care coordinator oversees and organizes the various aspects of our client’s healthcare and support services. This includes working with the nurses, social workers, and care managers to understand the client’s needs the is able to arrange medical appointments, coordinate transportation, manage medication schedules, and monitor the individual’s overall health and well-being, to report any concerns. They serve as the central point of contact for all involved parties, facilitating communication and scheduling. Additionally, care coordinators may provide education and support to families, as directed by care management staff. Overall, their goal is to make sure that appointments, meetings, and communication are managed and coordinated.
A private duty nurse is a skilled healthcare professional who provides personalized medical care and assistance to aging individuals in the comfort of their own home. You can expect a wide range of services offered, including medication management, wound care, monitoring vital signs, administering treatments, and coordinating with our team and their primary healthcare providers. Beyond medical support, they also offer emotional support and companionship, fostering a sense of security and well-being for the individual. Leaving familiar surroundings can be alarming and debilitating for older adults, particularly those with memory issues. Gentle Shepherd Care can gradually increase levels of care and expertise, starting with 4-hours of simple household management and moving up to 24-hour, nurse-supervised in-home care.
