Client Liaison, Eldercare Management (LPN required, RN preferred)
Client Liaison’s are responsible for coordinating and ensuring high quality of care. You will be responsible for:
- Assessing, evaluating and coordinating care around client needs (medical, personal, environmental)
- Consulting and coordinating with physicians and health care teams
- Advising on improvements to current care situations
- Evaluating, sourcing and contracting with health and home care vendors
- Supervising care delivery and vendor management
- Rapidly responding to and staffing for client events
- Responding to caregiver changes and outages, including training on client lifestyle and medical requirements
- Training new caregivers on protocols, policies and procedures
- Informing clients and family members in a timely manner of updates and changes
Credentials & Experience:
- LPN required, RN preferred
- 1+ years experience as a care manager

Nurse Specialist, Lifecare Planning (RN required)
Our Lifecare Planning team conducts individual assessments of clients’ current and future needs, and provides clients and their families with actionable plans. You will be responsible for:
- In person assessments of patients' health and health care needs
- In person assessment of patients' current mobility and adaptive needs
- In person environmental assessment and recommendations for improvements
- In person or virtual family resources evaluation, including financial, time, special skills
- Projecting into the future and formulating the most likely future needs
- Writing the clients' personalized Lifecare Plan and presenting plans to clients
- Assessing, capturing and writing formal Geriatric Assessments for legal presentations
- Updating Lifecare Plans as necessary
Credentials & Experience:
- RN required
- Experience with National Care Planning Council tools and procedures a plus
- Care management or discharge planning experience a plus

Nurse Supervisor, Care Delivery, At Home Care Services (LPN or RN required)
Our Nurse Supervisors of Care Delivery are responsible for ensuring the highest standard of care is delivered to our clients, including onsite assessments of care delivery team members. You will be responsible for:
- Establishing and reviewing care plans
- In person assessments of care delivery team members
- New employee training
- On site training and corrective measures
- Daily, weekly, monthly, quarterly and annual reports on caregiver actions vs. expectations
- On site adjustments followed by speedy documentation to better serve our clients
Credentials & Experience:
- LPN or RN required
- Experience managing teams of nurses and/or healthcare professionals preferred

Private Duty Nurse (RN required)
For select clients with extensive medical needs, Gentle Shepherd Care sources, qualifies and contracts with third-party, private duty nurses to deliver:
- Medication management
- Infusions, shots
- IV Hydration
- Post-hospital, post-rehabilitation care
- Chronic disease management
- Restorative, physical and/or occupational therapies
- Medical record updates
- End-of-life care (ie end-of-life doulas)
Credentials & Experience:
- RN preferred
- 1+ years experience in a skilled nursing facility, hospital or assisted living facility
- Basic Life Support (BLS) certification

Direct Care Giver, At Home Care Services (CNA or PCA required)
Direct Care Givers are the heart of our organization and the hands-on delivery of our promise to clients. You will be responsible for:
- Companionship - going beyond keeping our clients company to interacting with them in their areas of interest (news, gaming, puzzles, gardening, animals)
- Mental stimulation - ensuring our highly intelligent clients continue to exercise their brains every day
- Mobility, stability and physical activity -- each day, our clients need physical exercise, preferably out of doors; activity also can include restorative therapies
- Personal care -- supporting and delivering routine personal care (teeth brushing, dressing, changing, toileting) to ensure the happiness and dignity of clients
- Household care -- light housekeeping, grocery shopping, pet care, plant care
- Meal Preparation & Hydration -- to ensure good nutrition and hydration
- Medication Reminders -- to protect the health and medication compliance of all clients
Credentials & Experience:
- CNA or PCA required
- Basic Life Support Certification/ CPR Certified
- High School graduate or equivalent
- Comfortable using a computer
- References from home health positions a plus
- More than one year experience preferred
